Application Process:

in PDF Format,
Word Format, or
html and mail the completed form to or submit it in person at the address below.
Once you received a confirmation on your eligibility to apply for the CSEMS/S-STEM
Scholarship, you are required to complete your application package by submitting
all application materials listed at the bottom. Incoming freshmen and New Graduate Students: In Addition to the above form, download a printed version of the Application Form in PDF Fromat, Word Fromat, or html and mail the completed form along with all application materials listed at the bottom to the address below. All application materials should be enclosed in a single envelope if possible and mailed to: |

A complete application should include the following:
1. cover letter.
2. completed application form.
3. essay
of 500-1500 words discussing your career goals.
4. official high school
transcripts (freshmen).
5. all official college and university transcripts
(current students).
6. 2 sealed letters of recommendation.
7. official SAT scores,
unless they appear on a transcript.
8. other indicators which you feel should
be included (contests, presentations, community service, etc.).
9. signed
financial need release (on application form).
